Needs:

-NY State LPN License required

-Minimum of one year LPN Med/Surg experience in a hospital or Longterm Care facility required

- Must have end of life experience

-Must have a NY State Drivers License and a car

-Must be able to travel around the city

-Good computer skills required

Under the direction of an RN Case Manager, the LPN visits hospice patients in their home or in their facility. Following each patient's plan of care, the LPN provides direct care and documents each visit using the Netsmart EMR system; reporting any changes in the patient's condition to the RN Case Manager.
